Plan
of Care
The Plan of Care is a
stand-alone report that identifies the needs of an individual
and recommends options for addressing those needs in
an open and objective manner. Promoting continued wellness
of the individual is the main objective of the Plan
of Care, which is created for the benefit of the client
and the family to identify individual and family needs.
The report takes into account an individual’s
preferences, and analyzes the individual’s current
and past abilities, skills, routines and goals. It takes
into account the individual’s limitations and
barriers, and offers suggestions on minimizing them.
In preparing the Plan of Care, a Service
Coordinator will visit the individual in their home.
The Service Coordinator may also meet with other providers
and family members, in order to more fully understand
the individual’s needs. The report takes into
account other non-medical and medical services that
are currently utilized by the individual.
The Plan of Care is provided within ten
days of meeting the individual.
In addition to providing recommendations,
the report contains information about public and private
services that are available in the community. These
resources include contact names, telephone numbers,
websites, brochures and guide books.

7 days a week and are on a fee-for-service basis.
