Palliative Care Support
The palliative care support service is designed to provide psychosocial support to individuals that are living with a terminal or life threatening illness. This service eliminates the need for care in a hospital or alternative health care setting. It relieves caregiver stress and is a preventative measure based on the practices of psychosocial therapeutic support.
This service empowers individuals to remain in their home among their memories, family members and community. It is a respectful alternative for individuals and families faced with end of life care.
Services include:
- Providing Psychosocial care for the individual
- Co-ordination of medical care management and other services as needed
- Emotional and psychosocial support to family members, spouse, children and members of extended family
- Support the individual and family member in preparing for approaching death
